Supporting the improvement of patients' stay at our local hospitals

Summary of report content

Healthwatch Brighton and Hove took part in a workshop with University Hospitals Sussex NHS Foundation Trust in June 2023 to explore how to reduce unnecessary hospital stays and improve patients’ discharge experiences. The discussion formed part of wider NHS work to improve patient flow and discharge planning. Healthwatch drew on previous research showing that poor discharge planning can leave patients feeling unsupported and can contribute to difficulties managing at home after leaving hospital. 

A key message from the workshop was that discharge should not be treated as a separate event but as part of the patient’s entire hospital journey. Participants heard that remaining in hospital longer than necessary can increase risks such as falls, infection, deconditioning and poor mental wellbeing. The group agreed that planning for discharge should begin as soon as a patient is admitted, with clear arrangements for medication, therapies, support at home and transport.

Communication emerged as the most important area for improvement. Participants felt patients often receive too many forms, inconsistent information and unnecessary jargon. They recommended providing clear, accessible information about wards, visiting arrangements, discharge expectations and key contacts. They also emphasised the need for open communication between patients, families, carers and clinical staff, with discharge planning taking place collaboratively rather than being something that happens to the patient. 

The workshop concluded that earlier planning, clearer information and better communication could improve both patient experience and hospital efficiency. Suggested improvements included simple discharge information for patients, planning discussions involving families and carers, bedside information boards and shared discharge planning tools so everyone involved understands the plan and expected next steps.
