Healthwatch Wokingham Borough created this toolkit for GPs from a similar toolkit idea from Healthwatch Reading. It is a short signposting directory for caring related services in the area and is deliberately concise to ensure it is a 'quick read' for busy GP practices.

It includes information on the background for caring in Wokingham with statistics and brief case studies, as well as messaging in multiple languages. There are directory style listings for useful contacts and organisations that work with carers and clinicians.

It was created alongside Wokingham Borough Council and NHS Wokingham Clinical Commissioning Group.
