Summary of report content

Burton Street Foundation is a not-for-profit community organisation who provide a wide range of day services for people with learning disabilities, designed to develop self-esteem, self-confidence and self-awareness.

Healthwatch Sheffield worked with Burton Street Foundation to engage family members and support workers of adults with learning disabilities to find out more about their experiences.

152 individuals responded to the survey which sought to understand people’s knowledge of and experience with annual health checks and hospital passports, and any potential barriers to accessing them.

Key findings

  • Most people are offered annual health checks and do take them up, but there are still improvements to ensure everyone can benefit from these checks
  • Most would like a return to face-to-face appointments as a standard
  • People rely on GPs to remind them about appointments, so it is important that there is a system in place
  • Most people know what a hospital passport is but not everyone has been able to create one, more information and support is needed
  • Some feel that staff would not take the time to read hospital passports

A number of recommendations were made in this report to address the steps GP practices should take to increase uptake of annual health checks and for hospitals to increase awareness and use of hospital passports. For example, proactively inviting patients to appointments and making reasonable adjustments, and increasing awareness of hospital passports amongst staff through training.
