Healthwatch Brighton & Hove, Healthwatch East Sussex and Healthwatch West Sussex undertook a short survey to understand people's experiences of hospital discharge. 146 people responded.
Key findings
Half of respondents felt totally ready to leave hospital when they were discharged
The feature of discharge that was most frequently described as excellent was staff clearly explaining the process.
Nearly half rated their experience of discharge as excellent or good.
Respondents wanted to see the following changes:
- Improved communication with patients
- Ensuring appropriate post-discharge care has been appropriately considered and organised
- Faster dispensing of medication to minimise delays to discharge
- Improved involvement of family members and carers in discussions and planning for discharge
- Better support to assist patients (and carers) with the discharge process and any post-discharge needs
