Summary of report content

This is a report by Healthwatch Derbyshire. The report is based on engagement wt the discharge area of local hospitals.

A total 107 people gave feedback during engagement officer visits.

There were two main recommendations in the report:

• We recommend ‘Enter and View’ visits within the hospital discharge areas of Derbyshire hospitals to further understand the experience of patients leaving hospital. This may highlight areas that were not identified during  previous visits which took place on weekdays and during the daytime.

• The hospital trusts and Patient Experience Teams to review the effectiveness of the changes that have been planned and agreed upon following receipt of the patient experience.

The follow up action is to continue working with the trusts involved to improve services based on patient feedback. There is evidence of ongoing dialogue with two trusts.
