Healthwatch Cheshire West conducted an Enter and View visit to the Pharmacy at the Countess of Chester NHS Foundation Trust on the 5th of March 2015. The team undertook this Enter and View visit as part of a broader study of the whole discharge process at the hospital. The team spoke to staff and made general observations.
The findings indicated that there were a number of areas that could be improved to make the service more efficient. The report emphasised the importance of communication between departments to ensure services run smoothly and to ensure discharge is as efficient as possible. The team noted that the impression they received was that departments were generally working in isolation and that this was hindering their ability to run an efficient service.
The report made several recommendations. These included improving communication so that the pharmacy department are aware of ward closures and openings, and training up technicians to take more responsibility on the wards.
The report included a response from the service provider, acknowledging the recommendations and indicating that these were being explored.
