Summary of report content

In May 2016, Healthwatch Somerset carried out an independent engagement project to understand patients' and carers' experiences of the Early Supported Discharge (ESD) service for people recovering from stroke or brain injury. The project involved four focus groups held across Somerset and formed part of a wider evaluation of the service.

Healthwatch heard from 19 patients and 20 carers about their experiences of leaving hospital and receiving rehabilitation and support at home. Participants were generally positive about the expertise, commitment and responsiveness of ESD staff, and many described the service as helping to rebuild confidence and independence after discharge.

However, some participants highlighted challenges around communication, understanding what support would be available, and ensuring a smooth transition between hospital and community services. The findings were used to inform ongoing evaluation and development of the service across Somers
