Summary of report content

In May 2017, Healthwatch Somerset attended a meeting of the Compass Carers Support Group in Bishops Lydeard to hear carers' experiences of local health and social care services. The session formed part of Healthwatch's ongoing engagement with carers and included discussion of challenges encountered when supporting family members and navigating services.

Carers raised concerns about communication and information sharing between healthcare organisations, describing occasions where medical records, care plans or important treatment information had not been transferred effectively between hospitals. Participants also discussed concerns about the future of local carers' support services and examples of care coordination issues involving community services. Positive feedback was provided about support received through the Symphony Project. The findings were included in Healthwatch Somerset's reporting to local health and care partners to help inform service improvement.
