The key aim of the Community Navigation Pilot Service is to provide personalised information, advice and support to people using the service.
Background. 363 people used the service between March 2016-July 2017. Out of the 181 referrals made to the Community Navigation Service there were 105 connections/referrals made to over 25 different organisation/services.
Patients were interviewed by phone and were able to complete a survey.
92% of patients, interviewed as part of the evaluation, said that without the intervention of the service they would not have come across the services/activities on their own.
85% said the service was excellent or good.
The estimated saving based on 720 GP appointments which were avoided by signposting was £36,000.
Some recommendations were: That the service should be formally structured across 3 levels to formalise the types of support offered and to manage capacity within the service. A detailed review should be undertaken and recommendations produced for a plan to improve the way that Social Prescribing is able to access information about voluntary sector services.
