Summary of report content

In November 2014 Healthwatch Richmond conducted a review of ‘Compassionate Care’ at West Middlesex University Hospital. One of the most significant issues arising from this work was a concern amongst patients about the process of discharge from Hospital.  As a result, they decided to investigate hospital discharge further. They spoke to 120 people via surveys, interviews and outreach sessions.

Key findings:

  1. some patients did not feel that had been given the information they needed about discharge
  2. patients did not feel they had enough support following discharge
  3. patients experienced poor coordination between hospital and community services
