Carer experience of hospital discharge

Healthwatch Wandsworth was asked by NHS England to research the experiences of carers supporting people who are discharged since May 2021. They undertook 10 interviews with carers between 8th December and 22nd February 2022.
They found that:
- There needs to be a better way of identifying carers
- Co-ordinated and improved communication from the hospital to the carer was important
- Consistent information was needed about the care package and medication
- Carers weren’t always provided with the support they needed to care for the person coming out of hospital
- The importance of support from the GP
- Consideration of carer transport and hospital pharmacy delays
