Summary of report content

Healthwatch Blackburn and Darwen visited two care homes to speak to people, families and providers about their experiences of discharge from local hospitals. They utilised enter and view methodology as well as preparing questions to ask while there. They found that people were largely satisfied with their experiences of discharge but there were a small number of areas that could be tightened up.

Providers would like to receive notification of when residents were being discharged from hospital so that they can prepare appropriately, families wanted to be notified sooner and to not feel like the process is rushed and patients wanted to be assured that their discharge plans and personal effects would be sent with them.

Overall communication should be timely.
