Summary of report content

This is a report by Healthwatch Essex. The report looks at young people's experiences of transition to adult health and care services.

Both survey and interview methods were used to gather feedback. The project looks at whether young people felt supported during the change to adult services. Experiences of the transition and challenges faced are detailed.

This report does not have recommendations.

However in the conclusion the findings suggest there is greater need for support and understanding when young people are transitioning between child and adult services. A range of services are covered in this area and might include school support for educational needs as well as learning disability and or physical disability so there is considerable scope for a personalised approach.
