Summary of report content

Bracknell Forest Anticipatory Care Planning (ACP) and Healthwatch Bracknell Forest wanted to hear from residents as to what they would like, in their Anticipatory Care Plan.  They held a focus group on 24 March with 40 carers.

No one who attended had heard of anticipatory care planning before.  They suggested different names to better describe it.

People wanted to have conversations at home that built trust that their loved ones would be fully supported if they couldn’t care for them anymore.  They also wanted one point of contact.

They felt the carer, the care recipient and their family and friends should be included, as well as key professionals.  They came up with a list of issues the plan should include.

The report contains five recommendations about improving anticipatory care planning.
