Summary of report content

In May 2017, Healthwatch Somerset met with members of the Compass Carers Support Group in Martock to gather feedback about local health and care services. The discussion focused on carers' experiences of supporting relatives with ongoing health conditions and navigating hospital, ambulance and primary care services.

Participants described a number of concerns about communication between organisations, including care plans not being followed, medical records not being transferred between hospitals and information about ongoing treatment not being shared with GP practices. Concerns were also raised about access to GP services and hospital facilities, although carers highlighted positive experiences of support from specialist services including the heart failure team at Musgrove Park Hospital. The findings were shared with local health and care partners through Healthwatch Somerset's regular reporting processes.
